Salvage Vehicle Inspection Instructions



The following items are required at the time of the salvage inspection:
1. Wisconsin Salvage Title in owner’s name.

2. Pictures of the vehicle before it was fixed (at least 4) - front, rear, passenger side, and driver side. Please have printed color pictures.

3. A personal check for the $80.00 (plus $4.40 Fond du Lac County tax) inspection fee, plus any registration fees that may be required. Make one check payable to Registration Fee Trust. The check can be made out at time of inspection. Cash can not be mailed in.

You must have receipts or a Bill of Sale for any major parts (major parts statement form) that were replaced on the vehicle. Used parts must also have the V.I.N. of the vehicle that they were obtained from. Please make copies of receipts if you wish to keep them as the originals are sent in to the DMV.

5. Completed top portion (Sections A and B) of MV1, “Application for Title/Registration” http://www.dot.wisconsin.gov/drivers/forms/mv1.pdf

6. The person listed on the title as the owner of the vehicle must be at the inspection to sign the forms, OR have the MV1 form signed, and assign an Agent on the “Major Parts Statement” form to have the inspection done.

7. If the air bags were deployed in the vehicle, you need documentation that a diagnostic check of the air bag system was performed. If the vehicle’s frame was repaired or straightened you need documentation of a 4 wheel alignment which was performed on the vehicle. If the air bags did not deploy, or the frame was not damaged then a form will be signed stating such, at the inspection.

8. If you have license plates to transfer, please bring them to the inspection, so that the plate number can be transferred to the new title. You can drive to and from an inspection with out plates per State Statute. Do not put plates on vehicle before it has passed inspection as it is illegal to display unauthorized plates. This includes “Dealer” plates.

9. The inspection can not be performed if all of necessary paperwork is not brought to the inspection.

10. Please check all of the equipment of the vehicle before coming to the inspection: lights, windshield washers, air bag warning light, tires, parking brake, etc.

11. Inspections are performed at Car X in Fond du Lac, located at 470 N. Rolling Meadows Drive (across from Target). Car X charges a $40.00 plus tax lift fee, which is taken care of separately.

12. Inspections can also be done at a business or residence in Fond du Lac County if a hoist is available for the inspection.

13. Motorcycle inspections are done at the Fond du Lac County Sheriff’s Office,
as they do not need to be on a hoist.

Salvage Inspection Instructions if the registered owner will not be at the inspection

Prior to the inspection date:

Have the person named as owner on title:
1. Complete the MV-1 form (registration form) including the number of the license plate to be transferred if applicable. Make sure they sign the bottom
2. Fill out the bottom of the Major Parts Statement assigning someone who will be at the inspection as an agent. Make sure they also sign the top of the form indicating that they changed the major parts listed.

Dealers Only

• Dealers only can sell the vehicle at the time of inspection.
• Dealers fill out all inspection paperwork and an MV-1 form.
• Dealers only can re-assign owners on the back of the title.
• Dealers must fill out MV-11 form and have it signed by the buyer
• The title should be signed by the dealer and buyer on the back.
• If someone is acting as an agent then the dealer assigns them, not the new buyer. The only thing the new buyer fills out is the MV-11 form.
• Dealer has a check for the inspection, and the buyer should have a check for the MV-11 fees.
